Transcript Request Form 1 Use this form to request a transcript from your high school or college you previously attended be mailed to CCC&TI. Keep in mind that most colleges and universities charge a fee to process transcript requests. Check with your school for the appropriate fee and enclose payment with this form to avoid delays.
Transcript Request Form 2 Use this form to request that college personnel mail your CCC&TI transcript from Caldwell Community College to someone else.
