Admission Packets
Admission packets can be picked up at Continuing Education Department in Building H (Faye A. Broyhill) on Monday March 2, 2009 or download a PDF file below and print.
The admission Packet should be completed and returned as soon as possible. Students are considered on a first come, first served basis only. Every item on the checklist must be enclosed in your packet for it to be complete in order for you to be considered for a seat in the class. If incomplete, the packet will not be accepted.
Once the packet is returned to Corporate & Continuing Education, it will be dated and a number assigned according to the order in which received. This number will ensure an assigned seat in the class, provided all criteria are met throughout the entire admission process.
Patrick Benson, LMBT, Course Coordinator will review packet contents. Once he determines completeness, he will contact you to arrange a time for your interview. Please verify contact information on the registration form for accuracy. If there is a change of information at any time during the admission process, it is your responsibility to contact Patrick with any changes. Questions and/or concerns will be addressed during your interview.
Upon completion of the interview process, the student is then required to attend a mandatory orientation and registration session.
- Orientation and registration for Caldwell Massage Therapy Program is scheduled for October 27,2009 from 9:00a.m. until 1:00p.m. in the Faye A. Broyhill Building, room to be announced.
